The forms and information on this page can assist you in completing your FAFSA and other financial aid tasks. Most forms are not required of all students, and you will be notified by North Park University’s Financial Aid Office if you need to complete one of the forms listed here, such as for verification of your FAFSA. You will be directed as to what specific information must be returned to North Park. If selected for verification, you do not need to submit all of the forms listed below.

These forms apply to the 2024–25 academic year, which begins in August 2024. Your 2024–25 FAFSA was filed using 2022 income tax information.
You may need to submit one or more of the following worksheets if you are selected for verification for the 2024–25 academic year. You will be notified by the Financial Aid Office if you need to submit one of the forms below.

- As of January 14, 2019, we can also accept signed copies of the student and/or parent’s 2022 Federal 1040 as long as it includes copies of all W2s, and all schedules/attachments that go with the 2022 Federal 1040
- Form to request Confirmation of Non-Filing Status from the IRS. Do not list North Park as a third-party recipient when requesting your non-filing letter. You should have the IRS send the document directly to you; then you should submit a copy to North Park.
Parent PLUS Loan Change Form (online form for undergraduate students/parents): Parents who have borrowed money through a PLUS Loan for the current academic year and wish to make changes to the loan must submit this form to the Financial Aid Office to have those changes made. This form must be completed by the parent borrower of the PLUS Loan, not the student.
Church Matching (PIE) Grant Application: Churches may donate funds in your name, and North Park will match these funds up to $500 per year. This application must be completed by your church annually. (A separate Seminary Church Matching Grant Application is available here.)
FERPA Release Form: Once you enroll at North Park, your educational records will be protected under the Family Educational Rights and Privacy Act (FERPA). Release of your educational record is not allowed without written consent. This includes release to parents and family members. If you wish to have information released to a parent, guardian, or third party, please complete this form and return it to our office. Examples of educational records include, but are not limited to, your telephone listing, grades, billing/financial aid, discipline, and/or medical records.
Satisfactory Academic Progress Appeal Form: This form must be completed if you do not meet the minimum standards of Satisfactory Academic Progress, in which case, you will be notified by the Financial Aid Office.
Credit Balance Hold Form for STUDENT: This form must be completed if the STUDENT would like the credit balance (the result of having more aid/payments than charges on their account) to remain on their account, rather than receive a refund.
Credit Balance Hold Form for PARENT: This form must be completed by the PARENT if they would like the credit balance (the result of having more aid/payments than charges on the student’s account) to remain on their student’s account, rather than receive a refund.
Return Credit Balance Form: If there is a change to your enrollment that results in a credit balance (more aid/payments than what you are being charged), you can authorize the Financial Aid Office to reduce your loans by the amount of the credit, rather than receive the excess in the form of a refund. Reducing your loan will lower the amount that you have to pay back in the future.
Appeal Forms: If you need to provide additional information to the Financial Aid Office, such as unusual family expenses, lost income, or other factors that might reduce your and/or your family’s ability to pay your tuition bill, please complete and return the Appeal Intent Form to the Financial Aid Office at financialaid@cccbang.com. Please note, depending on the type of appeal you are pursuing, the financial aid office may request additional supporting documents.
